This time it wasn't a revert glitch. Two grangers built OM at the same time.
Pretty amazing.
I'm pretty sure I remember a bugzilla about that but I can't remember what was in the description to find it. You can break all kinds of game logic if you do things at exactly the same frame.

It is probably caused by ping.
I'm not sure about this, but building might be client-sided. If it is, then here's a solution:
One way to solve this is to reject a build request from the second client that builds when the first one is already building.
It wasn't the case of building OM while one was already building. The two grangers built OM precisely at the same time. You can check the time stamp on the game log.

i once reported the ability to deconstruct the last spawn by deconstructing the last two spawns in the same frame: https://bugzilla.icculus.org/show_bug.cgi?id=4059 (https://bugzilla.icculus.org/show_bug.cgi?id=4059). fixing that was rejected.
