well..depends. For example, with my cheapo nvidia 6200 i get the same fps, no matter what i do. Actually, it's getting better when i ENABLE trilinear filtering and real fade and stuff. So, i play at 1280x1024 with between 35 and 90 fps, depending on the map.
I'd suggest you simply try out which gives you the best results.

What are you're specs. Tremulous really isn't that intensive I get ~60-90 fps 720p, p4 3ghz. and 512mb ram.

Q3 is CPU bound, if your CPU is weak then graphic settings wont help

For max fps, type this in console :
r_picmip 5
It ll be activated next start of the game, but no more visible icons in score table.
r_picmip 2 is the best compromise imho

r_picmip garbage the scoreboard? ..but that sould be easy to fix!, you only need to patch the engine to ignore r_picmip while rendering the scoreboard, or better.. all 2D stuff!!.

No, you need to configure the shader coresponding to the scoreboard icons to ignore the picmip settings
