There are programs (though not a lot of free ones unless you search a bit) that you can use make freehand animated gifs with. As far as animated forum smileys like these go, it's a bit easier than a larger animation, as you're usually just changing several pixels at a time. I'm sure there are tutorials around for beginners.

Alternatively, they've got programs that will simply put individual static gif images together into an animated gif, but these aren't usually very useful.
